Birth Certificates

To obtain a certified copy of your Pennsylvania birth record or the record of an immediate family member, you are welcome to visit the Department of Health's web site at: www.health.state.pa.us or you may choose one of the following options:

1. By visiting one of the public offices located in New Castle, Pittsburgh, Harrisburg, Scranton, Erie or Philadelphia.
2. By sending your request via facsimile machine using a valid credit card.
3. By mailing your request, with the required fees, and the necessary information pertaining to the birth to:
